Purpose.Silverado Hospice - Houston South, a leading provider of end-of-life care, is presently seeking a LVN/LPN/charge Nurse to join our Hospice care team.Under the direction of a Registered Nurse, t he Hospice Licensed Vocational/Practical Nurse Field provides skilled nursing and palliative care for assigned hospice patients. He/she works cooperatively with the other members of the Interdisciplinary Hospice Team. The Hospice LVN provides intermittent visits or shifts of skilled nursing care, performs prescribed medical treatments, including pain management and symptom control, provides nursing observations, provides teaching regarding the expected course of the disease, care techniques and other health measures to the patient, family or other primary care giver, and participates in the care plan development and implementation. This is a non-exempt position. This position will work 8p-8a; but may be required to work day shift when necessary.ESSENTIALFUNCTIONS AND RESPONSIBILITIESUnderstands and exemplifies Silverados Vision, Mission, and Values. Understands and actively promotes Silverados LOVE>fear operating philosophy. Reports changes in the patients conditions and needs to the RN Case Manager or Director ofPatient Care Services. Administers prescribed treatments and medications as permitted by scope of practice. Provides those services requiring nursing skills in accordance with the plan of care. Educates and instructs the patient and patient's family. Demonstrates flexibility in patient scheduling and readily accepts new assignments. Observes for safety hazards in the patients and organizations environments and takes appropriate steps to reduce or eliminate them. Maintains continuing-ed through academic studies, seminars, and workshops. Attends and participates in in-services, staff meetings and case conferences. Completes all documentation in an accurate and legible manner. Complies with hospice policies and procedures, laws and regulations and standards of practice.
